El Síndrome del Túnel Carpiano; también conocido como el “Síndrome del Ratón”, es una de las enfermedades ocupacionales de mayor incidencia ya que es casi común que se presente en personas que realizan trabajos frente al ordenador por largas horas. Es una enfermedad que afecta a la muñeca, específicamente al paquete neuromuscular de esta zona anatómica causando inflamación  de la membrana sinovial y tendones que atraviesan el túnel carpiano; esto a su vez ocasiona compresión del nervio mediano. Lo que trae como consecuencia entumecimiento y hormigueo de la mano.

Causas del Síndrome de Túnel Carpiano

Existen diversas causas que dan origen al Síndrome del Túnel Carpiano y entre ellas se mencionan  las siguientes;

  • Flexión y extensión de forma repetitiva de la muñeca y la mano, como es la acción de teclear en el computador.
  • Realizar trabajos que requieran de prensión manual repetitiva,como las actividades rutinarias que hace un mecánico,ensamblaje, manufactura, costura, acabado, limpieza, y empaque
  • Realizar actividades de carga excesiva con las manos de forma inadecuada.
  • Enfermedades reumáticas.
  • La edad puede ser un factor predominante ya que se presenta entre los 40 y 50 años , siendo más frecuente en mujeres

Síntomas del  Síndrome de Túnel Carpiano

  • Dolor, entumecimiento y hormigueo a nivel de la mano.
  • Parestesia en los dedos índice y medio.
  • Debilidad muscular en la mano
  • Dificultad para realizar actividades que requieran de prensión, como hacer puño o pinza.

Diagnóstico del  Síndrome de Túnel Carpiano

Se realizan pruebas específicas para intentar producir los síntomas del síndrome del túnel carpiano.

  • Prueba de Tinel, el médico golpetea o presiona sobre el nervio mediano en la muñeca de la persona. La prueba es positiva cuando se produce hormigueo en los dedos o una sensación parecida a un shock.
  • La prueba Phalen, o de flexión de la muñeca, implica hacer que la persona sostenga sus antebrazos verticales apuntando los dedos hacia abajo y presionando juntos los dorsos de las manos. La presencia del síndrome del túnel carpiano se sugiere si uno o más síntomas, como hormigueo o aumento del entumecimiento, se sienten en los dedos en 1 minuto. Los médicos también pueden pedirles a las personas que intenten hacer un movimiento que produzca los síntomas.

Tratamiento del  Síndrome de Túnel Carpiano

De acuerdo al nivel de gravedad en que se encuentre la lesión, se podrá elegir el tratamiento adecuado para la sanación del paciente, pero siempre deben ir de la mano un equipo multidisciplinario como lo es la solución médica y terapéutica.

  • Medicamentos, esteroides y antiinflamatorios no esteroides, inyección con cortisona.
  • El uso de dispositivos, una férula a nivel de la muñeca.
  • Cambio de actividades, tanto en el ámbito laboral como en el hogar.

Fisioterapia en el Síndrome del Túnel Carpiano

La Rehabilitación Física estará enfocada durante las primeras sesiones, a disminuir  el dolor e inflamación que presente el paciente, mediante el uso de agentes físicos y técnicas terapéuticas.

  • Se aplica Crioterapia en las primeras sesiones de tratamiento por un tiempo de 15 minutos, en la zona donde haya dolor e inflamación.
  • Parafina en la mano afectada (sino hay inflamación) durante 15 minutos.
  • Se coloca Electroanalgesia con una duración entre 15 y 20 minutos.
  • Se realiza movilizaciones pasivas de la muñeca y mano lesionada.
  • Luego se le indica al paciente una serie de ejercicios terapéuticos, para mejorar las amplitudes articulares y aumentar la fuerza muscular de la mano y la muñeca.

Recomendaciones

  • Evitar actividades repetitivas y que requieran de un esfuerzo exagerado con las manos.
  • Utilizar una silla ergonómica, el escritorio a una altura adecuada, un mouse que genere comodidad y ergonomía; cuando se realicen trabajos frente al computador.
  • Implementar pausas activas (estiramientos musculares) durante las jornadas laborales, para así evitar enfermedades ocupacionales a futuro.
