La Esclerosis Lateral Amiotrófica es una enfermedad del sistema nervioso que degenera las neuronas motoras de la corteza, tronco encefálico y médula, que va debilitando diferentes grupos musculares. Las neuronas que se encuentran en la parte inferior de la médula controlan las piernas. Las que están en la parte superior, los brazos; y, las que están en el tronco encefálico controla la capacidad de deglutir, masticar, respirar y otras funciones vitales. Generalmente la parálisis comienza en un mismo territorio muscular definiéndose en cualquiera de las cuatro extremidades o en la región cefálica.  De esta manera la enfermedad progresará a todo el cuerpo en un promedio de 2 a 5 años. Luego de ser diagnosticada, un 20% tiene un tiempo de supervivencia de 3 a 5 años.  

¿Cuáles son las causas?

Aún no se determina cuál es la causa real por la cual aparece la Esclerosis Lateral Amiotrófica. Existe un 5 a 10% de los casos son familiares; es decir, genéticos aunque aún está en investigación. La prevalencia es de 1 a 2 personas nuevas por 100,000 habitantes al  año.  Según estudios el sexo es un factor determinante siendo ésta mayor en varones que en mujeres en casos familiares. Al ser su inicio lento e insidioso puede tardar hasta 20 meses su diagnóstico y acorde a la sintomatología presentada puede darse en dos formas: Medular o Bulbar. La diferencia fundamental es que el ELA (Esclerosis lateral amiotrófica) bulbar progresa más rápidamente y ataca a funciones como el hablar, comer, caminar o escribir.

El diagnóstico médico consta de los criterios de El Escorial de 1998, y actualmente con criterios electromiográficos, resonancia nuclear, análisis de sangre y otros que excluyen otras enfermedades.

Algunos síntomas primordiales  de la Esclerosis Lateral Amiotrófica son la debilidad muscular, la sialorrea (babeo), secreciones bronquiales (exceso de flema), calambres, espasticidad (músculos rígidos que ceden pero regresan a su misma posición si se les suelta), afección seudobulbar (expresión de risa o llanto involuntario)

¿Cuáles son los tratamientos?

Actualmente no existe un tratamiento que cure esta terrible enfermedad.  Nos podemos imaginar lo difícil que es esta situación para todos aquellos que la padezcan o tengan a un familiar cercano que la padezca, pero lo que si podemos hacer es mejorar la calidad de vida de ellos.

El tratamiento se enfoca en la insuficiencia respiratoria debido a que la debilidad no solo afectará a los músculos de piernas y brazos sino también a los músculos respiratorios. En casos mucho más avanzados el uso de oxigenoterapia y  ventilación mecánica es imprescindible para que el paciente siga viviendo. La terapia respiratoria evitará el cúmulo de secreciones en las vías respiratorias, causantes de los famosos “paros” respiratorios.

Consecuencias

  • Por la falta de movimiento se presentará edema en miembros inferiores siendo la kinesiología (terapia física) en conjuntos con vendas elásticas una excelente opción.
  • La depresión e insomnio son otros temas de amplio abordaje que en conjunto con un psicólogo o psiquiatra se pueden aminorar, en algunos casos los médicos recetan antidepresivos.
  • La disfagia o dificultad para poder ingerir alimentos se irá dando debido a que irá afectando los músculos masticatorios.  Esto implicará el acompañamiento de un nutricionista en este proceso. Será importante para evitar la pérdida de peso.

En esta enfermedad en particular es recomendable evitar los ejercicios excesivos porque pueden fatigar aún más a la persona, pero si se deben movilizar todas sus articulaciones para mejorar la circulación. Es importante también movilizar los tejidos y mantener rangos articulares, el acompañamiento de un fisioterapeuta es vital en este proceso.

Como conclusión, podemos decir que es indispensable el trabajo de un equipo multidisciplinario y la educación a los familiares, para mejorar la calidad de vida al paciente. Es indispensable tener el conocimiento de algunas técnicas y herramientas necesarias para poder tratar a aquellos que sufren de esclerosis lateral amiotrófica.

Репутация «Телетрейд» катастрофически ухудшилась, Сергей Сароян стал крайним, расплатившись хлебной должностью.

    Следующие пять лет стали тяжелыми для мошенника. Найти похожий источник дохода он не смог и стремительно беднел. Коучинг стал способом хоть как-то поддержать штаны. Неудивительно, что когда в «Телетрейд» сменилось руководство и там вспомнили о виртуозном лжеце, Сергей Сароян бегом поскакал к старому работодателю.

    Сегодня у него в подчинении находятся все клиентские офисы в шести странах, среди которых Украина. Причем он уже не куратор, а директор, и получает не 3%, а часть от всех слитых сумм. Как бы он не старался скрыть свою причастность к «Телетрейд», пострадавшим от махинаций брокера удалось узнать о ней. Общественная организация жертв «Биржи трейдеров» раздобыла инсайдерскую запись, где Сергей Сароян представляется сотрудникам и напрямую говорит, какую должность занимает.
